The Cooperative Extension System serves as the conduit through which scientific knowledge generated by the 130 land-grant colleges and universities in the United States is translated and delivered directly to its constituents. Since its inception over 100 years ago, Extension has been integral in developing, delivering, and applying cutting-edge knowledge in agriculture and natural resources, youth development, family and consumer sciences, and community and rural development. Today, more than ever, Extension will need to lead the way in building and maintaining (...) sustainable partnerships across disciplines and with organizations at the local, state, and national levels to tackle complex issues considering diminishing resources. Bringing together leading and emerging scholars, this volume discusses how Extension is addressing issues and opportunities relevant to children, youth, families, and communities across the country both now and in the future. A limiting factor in ethics education in medical training has been difficulty in assessing competence in ethics. This study was conducted to test the concept that content analysis of pediatric residents’ personal reflections about ethics experiences can identify changes in ethical sensitivity and reasoning over time. Analysis of written narratives focused on two of our ethics curriculum’s goals: 1) To raise sensitivity to ethical issues in everyday clinical practice and 2) to enhance critical reflection on personal and professional values as (...) they affect patient care. Content analysis of written reflections was guided by a tool developed to identify and assess the level of ethical reasoning in eight domains determined to be important aspects of ethical competence. Based on the assessment of narratives written at two times (12 to 16 months/apart) during their training, residents showed significant progress in two specific domains: use of professional values, and use of personal values. Residents did not show decline in ethical reasoning in any domain. This study demonstrates that content analysis of personal narratives may provide a useful method for assessment of developing ethical sensitivity and reasoning. (shrink)

Background In end-of-life situations, the phrase “do everything” is sometimes invoked by physicians, patients, or substitute decision-makers, though its meaning is ambiguous. We examined instances of the phrase “do everything” in the archive of the Ontario Consent and Capacity Board in Canada, a tribunal with judicial authority to adjudicate physician–patient conflicts in order to explore its potential meanings. Methods We systematically searched the CCB’s online public archive from its inception to 2018 for any references to “do everything” in the context (...) of critical care medicine and end-of-life care. Two independent assessors reviewed decisions, collected characteristics, and identified key themes. Results Of 598 cases in the archive, 41 referred to “do everything” in end-of-life situations. The phrase was overwhelmingly invoked by SDMs, typically to advocate for life-prolonging measures that contradicted physician advice. Physicians generally related “doing everything” to describe the interventions they had already performed, using it to recommend focusing on patients’ quality of life. SDMs were generally reluctant to accept death, whereas physicians found prolonging life at all costs to be morally distressing. The CCB did not interpret appeals to “do everything” legally but followed existing laws by deferring to patients’ prior wishes whenever known, or to concepts of “best interests” when not. The CCB generally recommended against life-prolonging measures in these cases, focusing on patients’ “well-being” and “best interests.” Conclusions In this unique sample of cases involving conflict surrounding resuscitation and end-of-life care, references to “do everything” highlighted conflicts over quantity versus quality of life. These appeals were associated with signs of cognitive distress on the behalf of SDMs who were facing the prospect of a patient’s death, whereas physicians identified moral distress related to the prolongation of patients’ suffering through their use of life-sustaining interventions. This divergence in perspectives on death versus suffering was consistently the locus of conflict. These findings support the importance of tools such as the Serious Illness Conversation Guide that can be used by physicians to direct conversations on the patients’ goals, wishes, trade-offs, and to recommend a treatment plan that may include palliative care. The main objective of this 2-week RCT study was to test the efficacy of the previously developed Noticing Nature Intervention to boost wellbeing during winter months. The NNI consists of noticing the everyday nature encountered in one’s daily routine and making note of what emotions are evoked. Community adults were randomly assigned to engage in the NNI or were assigned to one of two control conditions. Paired t-tests revealed significant increases pre- to post-intervention in the NNI group for positive affect, (...) elevation, nature connectedness, and hope agency, and a marginally significant increase in transcendent connectedness. No significant pre-post difference emerged for any aspect of wellbeing in the control conditions. Analysis of qualitative findings revealed that negative emotion themes were 2.13 times more likely to be associated with built photos than with nature photos. Feelings of peace, awe, happiness, humbleness, and hope were more likely to be associated with nature photos, while feelings of annoyance, loneliness, curiosity, uncertainty, anger, yearning, and comfortableness were more likely to be associated with built photos. Overall, results indicated that engaging in the NNI can provide a wellbeing boost, even in the cold of winter. This study is the first to test any nature-based wellbeing intervention during colder, winter months, and to directly assess the impact of a nature-based wellbeing intervention on levels of hope. Even in areas of philosophy of science that don’t involve formal treatments of truth, one’s background view of truth still centrally shapes views on other issues. I offer an informal way to think about truth as trueing, like trueing a bicycle wheel. This holist approach to truth provides a way to discuss knowledge products like models in terms of how well-trued they are to their target. Trueing emphasizes: the process by which models are brought into true; how the idealizations in (...) models are not false but rather like spokes in appropriate tension to achieve a better-trued fit to target; and that this process is not accomplished once and done forever, but instead requires upkeep and ongoing fine-tuning. I conclude by emphasizing the social importance of being a pragmatist about truth in order to accurately answer questions about science such as, “but do we really know that…”. (shrink)

Pushing back on policies favored by dying patients is a challenging endeavor, requiring tact, engagement, openness to bidirectional learning, and willingness to offer alternative solutions. It's easy to make missteps, especially in the age of social media. Holly Fernandez Lynch shares her experience learning with and from the amyotrophic lateral sclerosis (ALS) community, first as a caricature of an ivory tower bioethicist and more recently as a trusted advisor, at least for some. Not all intellectual property rights grant the right to exclude that is indicative of “property rules,” as that term was used by Guido Calabresi and A. Douglas Melamed in their seminal article. Some intellectual property rights are “liability rules,” in which the right holder has an entitlement to compensation for use of the protected invention, not a right to preclude the use. Although patent laws normally grant a right to exclude others from use of the protected invention as a default, (...) most countries’ laws allow the government to convert the patent property rule into a liability rule through a compulsory license. It has been noted, for example, that by the end of the 1950s, the U.S. had issued compulsory licenses covering 40 to 50 thousand patents, including substantial portions of the patent portfolios of AT&T, General Electric, IBM, and Xerox. The U.S. Supreme Court recently expressed a willingness to accept liability rules over injunctions in some patent infringement cases. (shrink)

Martin Hollis (d.1998) was arguably the most incisive, eloquent and witty philosopher of the social sciences of his time. His work is appreciated and contested here by some of the most eminent of contemporary social theorists. Hollis's philosophy of social action, routinely distinguished between understanding (rational) and explanation (causal). He argued that the aptest account of human interaction was to be made in terms of the first. Thus he focused upon the human reasons, for, rather than upon the (...) natural causes of, action. To help eliminate perinatal HIV transmission, the US Department of Health and Human Services recommends against breastfeeding for women living with HIV, regardless of viral load or combined antiretroviral therapy status. However, cART radically improves HIV prognosis and virtually eliminates perinatal transmission, and breastfeeding's health benefits are well-established. In this setting, pregnancy is increasing among American women with HIV, and a harm reduction approach to those who breastfeed despite extensive counseling is suggested. We assess the evidence and ethical justification for (...) current policy, with attention to pertinent racial and health disparities. We first review perinatal transmission and breastfeeding data relevant to US infants. We compare hypothetical risk of HIV transmission from breastmilk to increased mortality from sudden infant death syndrome, necrotizing enterocolitis and sepsis from avoiding breastfeeding, finding that benefits may outweigh risks if mothers maintain undetectable viral load on cART. We then review maternal health considerations. We conclude that avoidance of breastfeeding by women living with HIV may not maximize health outcomes and discuss our recommendation for revising national guidelines in light of autonomy, harm reduction and health inequities. Point-of-care testing (POCT) is a sensitive, specific and rapid form of testing for the presence of HIV antibodies. Post-exposure prophylaxis for HIV infection can reduce seroconversion rates by up to 80%. Needlestick injuries are the second commonest cause of occupational injury in the NHS and 20% of these occur during operations. In the NHS, in order to protect staff and patients from the risk of bloodborne viruses such as HIV, it is mandatory to report such injuries; however, numerous studies have (...) shown that many groups, particularly doctors, are reluctant to do so. This article outlines the arguments for and against the introduction of preoperatively seeking consent from patients to have their blood tested for HIV via POCT in order to improve the reporting rates of needlestick injuries incurred during surgery and to protect staff from infection. This research examines the extent to which similarities and differences exist in the codes of professional conduct of certified (chartered) accountants across the following countries: the United States, Taiwan, South Korea, Malaysia, Ontario (Canada), Australia, India, and Hong Kong. These eight countries exemplify some of the diversity in economic, political, legal, and cultural environments in which public accountants practice. The professional codes of ethics establish the ethical boundary parameters within which professional accountants must operate and they are a function of (...) these environments.The results of the study reveal that commonalities exist on some ethical rules indicating that some rules are indeed "culture free". Cross-country variations, however, exist as to the specificity and elaborateness of the rules. Such variations can be attributed to cultural and legal differences, as well as the length of time each professional organization has been in existence. An understanding of the similarities and differences in the codes is important to individuals who may work in these countries. Professional accountants involved in international business must understand the implications of the decisions they make in light of the ethical codes and moral values of their counterparts in foreign countries. After a discussion of the similarities and differences in the codes, the implications of these comparisons for accounting practice are discussed. (shrink)

In his article “Explanation Sketches” [3], Fred Newman argues that Scriven is mistaken in his interpretation of Hempel's notion of explanation sketch. According to Scriven, to complete an explanation is to fill in further initial conditions so that the explanandum of the sketch would be entailed by the new enlarged set of initial conditions. The point may be alternatively put by saying that according to Scriven to have a complete explanation is to have a covering-law which connects initial conditions with (...) the explanandum of the original sketch. Now Scriven's point against the deductive theory as applied to History is just that very often when we try to complete a given explanation sketch, we only succeed in unearthing a law which is either false or else trivial. Newman, however, contends that this is an excusable but, nevertheless, unsympathetic way of interpreting Hempel. He suggests an alternative account to the effect that to complete an explanation we need not necessarily produce a covering-law connecting initial conditions with the explanandum; all we need to do is to mention other initial conditions and certain laws which need not necessarily contain mention of the explanandum of the sketch. As he himself puts the point: “However, generously interpreted, when Hempel speaks of a sketch vaguely indicating laws we may take him as merely suggesting that some laws are vaguely indicated which would make up the explanans of an explanation of the event in question—not necessarily in the terms which describe the event in the sketch's explanandum.”. This paper is about the problem of explanation in anthropology. There are, broadly speaking, three theories of explanation, namely, the scientific theory, the historical theory, and finally what I have decided to call the phenomenological theory, after M. Natanson. The author argues that none of the three theories is adequate by itself to encompass the complex nature of anthropological science. The three theories correspond roughly to at least three different types of questions raised by anthropologists, and this being the case (...) the principle of methodological tolerance seems a natural and sensible principle to adopt. The paper also deals with the problem of reduction, i.e. the problem whether the three theories are different from and logically independent of one another. (shrink)

What kind of properties are moral qualities, such as rightness, badness, etc? Some ethicists doubt that there are any such properties; they maintain that thinking that something is morally wrong (for example) is comparable to thinking that something is a unicorn or a ghost. These "moral error theorists" argue that the world simply does not contain the kind of properties or objects necessary to render our moral judgments true. This radical form of moral skepticism was championed by the philosopher John (...) Mackie (1917-1981). This anthology is a collection of philosophical essays critically examining Mackie’s view. Zeno's famous paradox of the flying arrow, and the statements made in efforts to solve it by Hegel and Engels to the effect that a moving body is, at a given instant, both in and not in a given place, reveal, on the one hand, the objective contradictoriness of motion and, on the other, the difficulty of explaining it within the framework and method of formal logic. The elevation of the laws of formal logic to an absolute, and the exaggeration (...) of its role have led certain writers to deny the truth of Hegel's and Engels' statement. It was shown that no body can simultaneously be and not be in a given place, and that, in describing motion, one should, instead of employing this mode of expression, merely say that the body is shifting, and successively passes one point after another. On one hand, the extensive application of data science to healthcare research is progressing faster, while on the other hand, the ethical concerns and adjoining risks and legal hurdles those data scientists may face in the future slow down the progression of healthcare research.
